What’s a Care Team?
Core Team
Every member is assigned a core care team. These are professionals involved in every aspect of your care. Your primary care provider (PCP) leads the team, which includes two nurses, a social worker, and a medical assistant. Their compassionate, nurturing manner helps you feel secure.
Integrated Medical Team
If you need a prescription, your onsite pharmacy will fill it. Need bloodwork? Your onsite lab will draw it. Nearly everything you could want from a medical team is here. With all your care under one roof, we are always connected to and coordinated with you.
Integrated Life Services Team
Sometimes the stress of life affects our health. We’re here for that, too. Our behavioral specialists can help with anxiety and depression. Our social workers can help you with some day-to-day needs. Our pharmacy delivers, and the driver is part of your life services team.
