Our Care Model
AbsoluteCare engages our members in their own communities—where we all shop, live, and work—to find the people who need us most. Our members are at higher risk for, or have, multiple chronic illnesses. They often rely on the impersonal, understaffed emergency rooms for even the most basic of diagnoses and medications.
Our goal is to foster trust with each individual member and remove them from a medical system that is equipped primarily to treat one set of symptoms at a time.
Our care model puts the emphasis on care.
Each member has a core care team—a group that includes:
• Primary care provider (PCP)
• 2 Nurse (RN) care managers
• Licensed clinical social worker (LCSW)
• Medical assistant
Our integrated teams strengthen the core. Integrated services provide a protective outer layer that nurtures the whole person:
• Substance abuse
• Clinical pharmacy
• Radiology
• Nutrition
• Behavioral Health
• Transportation
• Other medical and life services
Our community-based teams and services include:
• Community health workers
• New member enrollment specialists
• Transitional care managers
• Home-based RN care managers
Our care model works.
Smaller member panels with fewer daily encounters help us to reduce waste, enhance member compliance, improve trust, and help reduce the burden on overstressed healthcare resources. But the real proof is in our members.
